FACILITATED ENROLLMENT FOR CHILD HEALTH PLUS, FAMILY HEALTH PLUS AND MEDICAID

The Facilitated Enrollment Program, a NYS DOH initiative, was implemented in April 2000 by NCPPC. This year the three northern counties were provided enrollment services through 2 subcontracts to community agencies in the tri-county area (North Country Children's Clinic and Planned Parenthood of Northern New York) and through NCPPC. Families receive assistance with the Medicaid, Family Health Plus, and Child Health Plus application process. Since the beginning of the new contract in January 2003, 10,504 children and 4,292 adults have been enrolled in the tri-county area.
